Experienced live-in caregiver needed for 90-year-old gentleman with slightly limited mobility in a lovely 3-bedroom creekside central Catskills home with mountain views. Duties include companionship, assistance with personal hygiene including bathing and shaving, dressing, transfer assistance, administering medications, assistance with prescribed exercises, meal preparation, housekeeping, laundry, scheduling doctors' appointments, driving to those appointments, shopping and other errands.

Caregiver will work four days a week, Noon Tuesday to Noon Saturday, with the possibility of a fifth day being added in the future. The caregiver will have a fully-furnished bedroom suite with private bath, cable TV, and broadband internet service. Most mornings will be free for personal errands. Caregiver will be expected to have some other place to stay on days off since remaining on premises on those days won’t be a viable option.

Candidate must be able to provide care with kindness and compassion, have a calm and professional demeanor, be skilled in the art of gentle persuasion, speak and read English, be a non-smoker, provide references and pass a background check. HealthPort Technologies, LLC., is currently seeking a Medical Records ROI Specialist to travel to local healthcare facilities in Saratoga Springs, NY and surrounding areas to process medical records requests.

This position is responsible for processing all release of information requests in a timely and efficient manner ensuring accuracy and providing customers with the highest quality product and customer service. Associate must at all times safeguard and protect the patient’s right to privacy by ensuring that only authorized individuals have access to the patient’s medical information and that all releases of information are in compliance with the request, authorization, company policy and HIPAA regulations. This position will require travel to various client sites during the course of the day.

This is a part-time day shift position Monday through Friday. Approximately 15-20 hours per week. This position requires daily local travel.

Essential Duties and Responsibilities:

Completes release of information requests including retrieving patient’s medical chart and returning chart, scanning medical record accurately and correctly and transmitting daily, according to requests, established procedures, and established standards of quality and productivity.
Date stamps all requests and highlights pertinent data to facilitate processing.
Validates requests and authorizations for release of medical information according to established procedures.
Performs quality checks on all work to assure accuracy of the release, confidentiality, and proper invoicing.
Maintain equipment in excellent operating condition (inside and out) and troubleshoot equipment issues with assistance from the Help Desk department.
Provides excellent customer service by being attentive and respectful; insures understanding of customer request and follows-through as promised; and being proactive in identifying client concerns, or problems.
May receive incoming requests including opening mail, assisting walk-ins and telephone inquiries, and retrieving facsimile inquiries, depending on the needs to the client.
Maintains a neat, clean, and professional personal appearance and observes the dress code established.

Complete and send in SWAR weekly activity reports at the end of each payroll cutoff

Carries out responsibilities in accordance with HealthPort and client/site policies and procedures, including HIPAA, state/federal regulations related to operations, and labor regulations.
Maintains confidentiality, security and standards of ethics with all HealthPort and medical records information including transport, storage and disposal.
Work with privileged information in a conscientious manner while releasing medical records in an efficient, effective, and accurate manner.
Performs other tasks as assigned including but not limited to the requirement to work at facilities in close proximity to your principal place of business as directed by the Service Specialist, Area Manager or District Manager when business needs dictate.

Exciting opportunity for an RN to work with patients diagnosed with CHF, COPD, Diabetes and/or End Stage Rental Disease and who have been readmitted to the hospital within 30 days of their discharge. The RN will meet with the patients in the hospital and follow them as they transition to inpatient rehab and then conduct a post discharge home visit to review medications and assess for rehospitalization risk factors. They will accompany patients with complex chronic illnesses to their first MD appointment after discharge and follow-up with the patient/caregiver via phone for a total 30-day period post discharge. Coaches will work as part of a Project Team including acute care case managers, home care liaisons, rehab facility staff, pharmacists and hospitalists.

Candidates are required to have a BSN, strong patient/caregiver teaching skills, the ability to work effectively as part of a team and to collect outcome data. Home care experience/knowledge of community resources preferred. Valid nursing and driver’s license required. Must be willing to travel within the Capital Region. The class is a total of 8 days and is not paid but it is FREE. Class begins promptly at 8:30am and ends at 4pm. After passing the class and fulfilling your work requirement you will be presented with a PCA certificate in addition to a certificate documenting your completion of our Traumatic Brain Injury training program. Qualifications for this class are as follows;

Car and CLEAN Driver’s License Required*

Ability to pass a criminal background check*

Compassion for those with different disabilities*

An interest in working for our company*

Ability to attend all 8 days of class on time*

This is a NYS certified position providing personal care and light housekeeping tasks for members of the community in their own homes. Cases range from an hour or two to sometimes longer, so travel between cases is necessary. Once you have established your availability, we will set up your schedule.
