REPORTS TO: PATIENT CARE COORDINATOR / DIRECTOR OF CLINICAL SERVICES
STATUS: EXEMPT
GENERAL PURPOSE: Manages overall care of assigned hospice patients on all levels of care by coordinating and providing nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. Supports the organization’s work in the community including marketing and volunteer activities.
ESSENTIAL JOB FUNCTIONS:
COMMUNICATION Communicates with attending physician, Hospice Medical Director, members of the IDT, and other agencies as needed to coordinate optimal care and use of resources for assigned patients/families. Maintains regular communication with the Patient Care Coordinator and Director of Clinical Services.
NEEDS ASSESSMENT Receives patient assignments from the PCC or DOCS. Assumes case management responsibilities for assignment including assessing, planning, implementing, reevaluating, and revising care as necessary. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.
CASE MANAGEMENT Coordinates all services provided to patients/families in all settings to ensure quality of care/services as well as appropriateness of care.. Supervises implementation of the approved plan of care.
PATIENT RECORDS Maintains current patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur. Initiates and sustains the implementation of orders for medications and treatments as prescribed by the physician in the plan of care. Completes electronic point of care charting on patients/families. Participates in clinical audits as assigned by the PCC or DOCS and completes tasks requested to remain in compliance with federal and state regulations.
COORDINATING PATIENT CARE Accepts responsibility for managing overall care of the patient by providing direct patient care, and by coordinating the care of the patient by teaching primary caregivers, volunteers, and employed caregivers as appropriate. Attends all team specific IDT meetings presenting his/her patients and making changes in the POC as agreed to by the IDT. Attends care conferences as required by LTC regulatory policy. If assigned, provides direct supervision once a month and as the need arises.
PATIENT/CAREGIVER INSTRUCTIONS Instructs the patient and caregiver how to administer medication and recognize side effects; how to perform personal care and oral hygiene; how to assist in lifting, moving, ambulation, and exercise; how to prepare for the events preceding death; proper diet; and other instructions as ordered by physician. Updates POA weekly on patient status.
FAMILY AND PATIENT SUPPORT Informs the DOCS and PCC of unusual or potentially problematic patient/family issues. Makes death pronouncements and provides appropriate support to family. If requested, shares in on call responsibilities to assure 24-hour, seven-day-a-week coverage to patients and families.
HOSPICE AIDE Establishes Hospice Aide Care Plan, communicates changes in POC and makes supervisory visits as required by regulatory agencies.
EVALUATIONS/ ADMISSIONS Completes evaluations and admissions as assigned by the Intake Coordinator, PCC, or DOCS.
POLICY AND PROCEDURE Demonstrates familiarity with policies of the hospice and state and federal regulations pertaining to nursing services. Responsible for adhering to conditions of participation as outlined by regulatory agencies.
CODE OF CONDUCT AND BUSINESS ETHICS Must adhere to Code of Conduct and Business Ethics policy including the documentation and reporting responsibilities.
QAPI Participates in the Hospice’s Quality Assessment and Performance Improvement Program and Hospice sponsored in-service training.
CONTINUING EDUCATION Hospice personnel are expected to participate in orientation, ongoing educational programs and appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs. Maintains required contact hours/continuing education units as required for licensure/certification.
OTHER Participates on committees and special projects and performs other duties as assigned. Accepts responsibility of assignments to perform specialized procedures, such as IV therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.
PHYSICAL AND SENSORY REQUIREMENTS (WITH OR WITHOUT THE AIDE OF MECHANICAL DEVICES): Mobility, reaching, bending, lifting, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, ability to see up close, and ability to remain calm under stress.
QUALIFICATIONS:
• Registered Nurse currently licensed in the state.
• Must have a minimum of one year general nursing experience with current medical, surgical, or critical care experience.
• Must demonstrate a commitment to the hospice philosophy of care.
• Must be knowledgeable of Medicare hospice certification.
• Experience in providing services to terminally ill patients preferred.

Gallup Dialysis Center is looking for a qualified Social Worker. The Social Worker is a person who oversees the psychosocial care and support of patients with chronic renal disease who are undergoing renal replacement therapy.
Orientation of the new patient and family to dialysis:
• The dialysis social worker meets with the prospective patient and his/her family to assist with an initial understanding of renal disease and dialysis as treatment, and offers emotional support to them as needed.
• The LCSW completes an initial psychosocial assessment of the patient within 30 days of admission, which is filed in the patient's medical record.
• The concept of Advanced Directives is reviewed with the patient and family. Any concerns regarding the patient's emotional state are discussed with the attending physician and Administrative Nursing Director.
• Any immediate needs, such as transportation to/from the clinic, are addressed as soon as possible upon/ after admission to the facility.
• The long-term Life Plan is completed within the first 30 days of admission. Options for treatment of ESRD are reviewed with the patient, and the social worker acts as liaison with the physician to provide all necessary information regarding modalities to the patient.
• The HCFA 2728 form is completed per Network and unit specifications; a screening of eligibility for Medicare and/ or Medi-Cal is performed when appropriate.
On-going psychosocial support of ESRD patients including, but not limited to:
• Helping the patient to achieve an optimum level of function, utilizing available resources in the facility, family, and community.
• Addressing recurrent issues such as: insurance eligibility/ coverage, transportation provisions, vocational rehab, and ESRD acceptance.
• Assisting the patient in changing the treatment modality (e.g. transplant), or permanent transfers to other units.
• Being available to patients with special psychosocial needs and available to patients upon patient request.
Record Keeping:
• Responsible for making entries in the patient's medical record in accordance with the requirements as established in the Medicare Conditions of Coverage as a minimum.
• Short-term care plans are reviewed at least semi-annually for all patients and monthly for unstable patients.
• Long-term Life Plans are reviewed at least annually per Network requirements.
• Conducts Patient Satisfaction surveys at least annually and Quality of Life surveys as appropriate and reports findings to the Quality Improvement Committee.
