Case Manager - For the elderly (Albuquerque)

Home Care Assistance is hiring a qualified full time Case Manager to work with its elderly population. The Case Manager must have at least 3 years' experience in the home health field, be compassionate, caring, diligent, and punctual. Excellent communication skills and computer literacy are also required.

The position requires rotating ‘on call’ supervision for caregivers and new clients which may require emergency PRN shift fulfillment and scheduling using software.
