Duties/Responsibilities: Working closely with the area leads, supervisors and the department manager, the night shipper coordinates all daily shipments to ensure things are complete and loaded. Night shipper position assists efforts within and among various work areas of the shop in ensuring Quality, Cost, and Delivery of Unirac products. The night shipper is an expert in the loading of Unirac materials onto the various types of trucks used (LTL, Van, Flatbed). The night shipper must also be an expert in related UFP’s.

JOB TITLE: REGISTERED NURSE CASE MANAGER
REPORTS TO: PATIENT CARE COORDINATOR / DIRECTOR OF CLINICAL SERVICES
STATUS: EXEMPT


GENERAL PURPOSE: Manages overall care of assigned hospice patients on all levels of care by coordinating and providing nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. Supports the organization’s work in the community including marketing and volunteer activities.

ESSENTIAL JOB FUNCTIONS:

COMMUNICATION Communicates with attending physician, Hospice Medical Director, members of the IDT, and other agencies as needed to coordinate optimal care and use of resources for assigned patients/families. Maintains regular communication with the Patient Care Coordinator and Director of Clinical Services.

NEEDS ASSESSMENT Receives patient assignments from the PCC or DOCS. Assumes case management responsibilities for assignment including assessing, planning, implementing, reevaluating, and revising care as necessary. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.

CASE MANAGEMENT Coordinates all services provided to patients/families in all settings to ensure quality of care/services as well as appropriateness of care.. Supervises implementation of the approved plan of care.

PATIENT RECORDS Maintains current patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur. Initiates and sustains the implementation of orders for medications and treatments as prescribed by the physician in the plan of care. Completes electronic point of care charting on patients/families. Participates in clinical audits as assigned by the PCC or DOCS and completes tasks requested to remain in compliance with federal and state regulations.

COORDINATING PATIENT CARE Accepts responsibility for managing overall care of the patient by providing direct patient care, and by coordinating the care of the patient by teaching primary caregivers, volunteers, and employed caregivers as appropriate. Attends all team specific IDT meetings presenting his/her patients and making changes in the POC as agreed to by the IDT. Attends care conferences as required by LTC regulatory policy. If assigned, provides direct supervision once a month and as the need arises.

PATIENT/CAREGIVER INSTRUCTIONS Instructs the patient and caregiver how to administer medication and recognize side effects; how to perform personal care and oral hygiene; how to assist in lifting, moving, ambulation, and exercise; how to prepare for the events preceding death; proper diet; and other instructions as ordered by physician. Updates POA weekly on patient status.

FAMILY AND PATIENT SUPPORT Informs the DOCS and PCC of unusual or potentially problematic patient/family issues. Makes death pronouncements and provides appropriate support to family. If requested, shares in on call responsibilities to assure 24-hour, seven-day-a-week coverage to patients and families.

HOSPICE AIDE Establishes Hospice Aide Care Plan, communicates changes in POC and makes supervisory visits as required by regulatory agencies.

EVALUATIONS/ ADMISSIONS Completes evaluations and admissions as assigned by the Intake Coordinator, PCC, or DOCS.

POLICY AND PROCEDURE Demonstrates familiarity with policies of the hospice and state and federal regulations pertaining to nursing services. Responsible for adhering to conditions of participation as outlined by regulatory agencies.

CODE OF CONDUCT AND BUSINESS ETHICS Must adhere to Code of Conduct and Business Ethics policy including the documentation and reporting responsibilities.

QAPI Participates in the Hospice’s Quality Assessment and Performance Improvement Program and Hospice sponsored in-service training.

CONTINUING EDUCATION Hospice personnel are expected to participate in orientation, ongoing educational programs and appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs. Maintains required contact hours/continuing education units as required for licensure/certification.

OTHER Participates on committees and special projects and performs other duties as assigned. Accepts responsibility of assignments to perform specialized procedures, such as IV therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.

PHYSICAL AND SENSORY REQUIREMENTS (WITH OR WITHOUT THE AIDE OF MECHANICAL DEVICES): Mobility, reaching, bending, lifting, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, ability to see up close, and ability to remain calm under stress.


QUALIFICATIONS:

• Registered Nurse currently licensed in the state.

• Must have a minimum of one year general nursing experience with current medical, surgical, or critical care experience.

• Must demonstrate a commitment to the hospice philosophy of care.

• Must be knowledgeable of Medicare hospice certification.
• Experience in providing services to terminally ill patients preferred.

• Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements.

• Must demonstrate excellent observation, nursing judgment, and communication skills.

• Must possess the ability to provide good written documentation in a timely manner.

• Must demonstrate ability to use electronic charting and maintain point of care charting.

• Must be self-directed with the ability to work with minimal supervision.

The project supports the work of Elev8 New Mexico, which functions as a full-service community school and is part of a national initiative that serves middle school youth and their families, providing them with a powerful combination of integrated services and supports on school campuses that leads to their success in school and in life. These services consist of Extended Learning, Family Supports, Family Resource Centers and School-Based Health Centers. Building on the capacities of local and national partner agencies, Elev8 New Mexico’s evidence-based approach assures high-quality health, learning, and family supports before and after school, on weekends, and during the summer. The ultimate benefit of an integrated approach that includes academic supports, social services, family supports, parent organizing, mentoring, quality out-of-school time opportunities, and comprehensive school-based health leads to improved student learning, stronger families, and healthier communities. The project empowers families to advocate for sustaining programs that help guide their children in facing the complex challenge of transitioning from childhood to adulthood. The project also empowers families to collaborate with other community groups in advocating for these programs as well as advocating for other issues of concern to the education and well being of their children.

Community and Parent Outreach 75%
Enlist parent participation in enrolling their children in Elev8 programs.
Recruit and organize parents, community organizations and community members in advocating for sustaining and replicating Elev8/a full-service community school model (and the 3 components: extended day learning, school based health centers and family supports) on a local, state and national level.
Recruit and organize parents, community organizations and community members in advocating for issues of concern to middle school students and their families as identified by parents and the Elev8 team.
Participate in the planning and implementation of community and parent outreach campaigns through home visits, one on one and group meetings, phone calls and flier and email distribution and other methods.

Research, Evaluation and Assessment 15%
Assist the Elev8 Team with the development and refinement of evaluation and assessment tools.
Record parent and community contacts and the action they took toward achieving Elev8 goals.
Along with the Elev8 team, analyze and assess the successes and challenges of each campaign using recorded data and anecdotal experience.
Adjust strategies during a campaign and while planning new campaigns based on recorded data and anecdotal experiences.

Professional Development 5%
Participate in State Office sponsored A*VISTA trainings.
Participate in training opportunities offered by Elev8 and its stakeholders.

General Office Management 5%

UP-TO-DATE IMMUNIZATIONS REQUIRED AND WILL BE REQUIRED TO SHOW RECORD PRIOR TO EMPLOYMENT.

Need a Port a Barn put up (Edgewood)

I have a used 3 stall, in line, shed row port a barn that needs to be put up. It is a bolt up type, but will need a cement footing put in. You must be very handy, have the know how, & tools, I will have all the parts, and if we need more, you must have the transportation to go the lumber store to get what we need, at my cost.
