Psychiatric Nurse Practitioner (Ann Arbor MI Area)

We are currently seeking a permanent, full-time psychiatric Nurse Practitioner near Ann Arbor, Michigan. The NP will cover both inpatient (adult) and outpatient work as part of a psychiatrist group. The inpatient census averages 8 patients/day for a five hour shift with 3 hours of out-patients. Outpatient evaluations are one hour and follow-ups are 20 to 30 minutes. Call schedule is 1 in 4 weekends and nights, by phone except for weekend rounding (approximately 2 to 3 hours on Saturday and Sunday).

Life Enrichment Coordinator Full-time (Ann Arbor)

Glacier Hills has been an integral part of the greater Ann Arbor community for more than 35 years, providing outstanding customer service and quality care to our 400+ older adult residents. Our acclaimed Care and Rehabilitation Center at Glacier Hills provides both long-term care and short-term rehabilitation person centered care to older adults. We are seeking a fuill-time LIFE ENRICHMENT COORDINATOR to plan and implement meaningful programs, classes, and events for our long-term care residents. This position must embrace and demonstrate the value of “living in the moment" for individuals with memory loss.

DUTIES
Additional responsibilities will include team building with others; providing relationship centered care; recording and maintenance of required state and federal documentation; facilitating resident trips; coordination and instruction of volunteers in resident centered events and attendance at weekly care plan meetings as assigned. Coordinates and works recreation programs within other departments as needed.

QUALIFICATIONS
Bachelor’s or Associate’s Degree in a health related field.
Experience in programming/planning activities for individuals with memory loss.
Excellent customer service and communication skills.
Proficient in MS Office programs.

Physical Therapist Assistant (PTA) Full-time Days (Ann Arbor)

Provide care per physician's orders and plan of care developed by evaluating therapist.

Take measurements of resident status as appropriate in providing care. Assist in assessing the effectiveness of care.

Maintain communication and collaboration with the evaluating therapist to promote maximum benefits from therapy. Summarizes resident performance according to established goals.

Implements education plan as outlined by evaluating therapist, determines additional educational needs of the resident's family/caregiver/significant other as they arise and in collaboration with the evaluating therapist.

Follows plan for discharge and discusses with evaluating therapist any additional discharge concerns that may arise. Assists therapist in goal reassessment and discharge planning.

Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient/client populations served.

Ensures appropriate and accurate billing practices that impact the delivery of services.

Assesses educational needs (with evaluating therapist) of the patient, family or caregiver and provides education to meet those needs.

QUALIFICATIONS

Associate's Degree as a Physical Therapist Assistant from an accredited training program approved by the American Physical Therapy Association.
