Area Director Clinical Documentation Improvement Specialist (Southern California, CA)

Position Summary:

This is an Area Director level position, which supports multiple long-term acute care hospitals within the program for clinical documentation improvement. This position uses masters level project management, clinical knowledge and knowledge of coded data for documentation requirements to improve overall patient quality, capture severity, acuity and risk of mortality. Expertise required in understanding the clinical documentation required for the completeness of the patient records using a multidisciplinary and interdisciplinary team process. Provides oversight, leads and directs an Area (within a Region) clinical documentation improvement program and staff. Position includes recruiting, hiring and supervising RNs and/or qualified HIM coding professional(s) who are CDI staff. Works with hospital and Regional/District team to reach goals and objectives of the program, which include quality, safety, data integrity and accurate reimbursement. This position functions within the (West) Regional Case Management Department.



Education/License/Certification:

Graduate from an accredited school of nursing (BSN required) and/or medicine and/or experience and Training applicable to clinical and CDI job experience (Physician Assistant, Nurse Practitioner, RHIT) If RN, current licensure to practice as a Registered Nurse in the State of California. Masters Degree preferred. CCDS or CCS optional.


Qualifications:

· Minimum of 6-8 years clinical experience (i.e. inpatient, clinical documentation, and/or case management reviews.

· Prior Clinical Documentation Improvement experience, either as a specialist or as a program manager/leader.

· Expert interpersonal, communication (verbal, non-verbal, and listening skills).

· Understand Adult Learning Theory.

· Understand coding classifications systems such as, but not limited to ICD-9 CM, MS-DRG, APR-DRGs, and HCCs strongly preferred.

· Strong ability to present to multiple physician advisory boards, steering committees and executive staff on the importance of documentation improvement, and effectiveness of CDI and its’ goals and objectives.

· Collect, analyze and interpret data (CDI) for physician endorsement of program initiatives.

· Facilitate understanding, compliance, and completeness in documentation.

· Ability to combine clinical knowledge and business acumen to deliver results with a track record of assessing and capitalizing on opportunities for streamlining operations through broad based experience in project leadership and process reengineering.

· Consistently produce client deliverables faced with multiple project resource constraints.

· Utilize training in Six Sigma methodologies.

· Must exhibit efficiency, collaboration, candor, openness, and results orientation.

· Knowledge of the healthcare revenue cycle.

· Competent computer skills including word processing, spreadsheets, and presentation software.

· Must have strong analytical skills. Demonstrate an understanding of the operations and/or business of Healthcare LTCH, health policy trends, and any applicable regulations related to the responsible practice area.

· Experience with Recovery Audit Contractors (RAC).

Preferred Qualifications:

· Acute care inpatient care experience and extensive knowledge of MS-DRG, APR-DRGs or similar methodology.

· Knowledge of inpatient clinical documentation issues preferred.

· CCDS (certified clinical documentation specialist), CCS (Certified Coding Specialist) optional.


Duties:

· Implement and provide oversight for an Area Clinical Documentation Improvement Program in an organized and standardized manner.

· Recruit, hire and train clinical documentation improvement staff that will function at the District hospitals.

· Mentor and supervise CDI staff.

· Establish effective working relationships with the local and regional staff/teams/leadership.

· Facilitate appropriate clinical documentation to support diagnosis capture.

· Identifies and reviews primary and secondary diagnosis and complications to ensure diagnosis and Identify and review for POA (Present on Admission), Hospital Acquired Conditions (HACs) documentation and initiate a communication clarification process when appropriate with providers.

· Reviews clinical issues with medical coding staff and with physicians to identify those diagnoses that impact severity of illness indicators) for each patient.

· Serves as an expert resource in reviewing all medical records in support of consistent documentation for all payer types (i.e. CMS, Medicare-Advantage, RACs, etc) to ensure complete and accurate diagnosis capture and coding.

· Adheres to the hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.

· Collaborates in the development of programs, initiatives and workflows, which provide alignment with education for internal customers to support clinical documentation guidelines.

· Communicates information timely and effectively with medical center leaders and clinical staff.

· Works withCase Management Corporate and Regional Senior Directors to develop, implement and monitor departmental policies and procedures that support organizational goals, business objectives, regulatory needs and requirements.

· Conducts quality assurance reviews on the CDI processes and functions, resulting in the reporting of any corrective action.

· Collaborates with the interdisciplinary contacts of the medical centers; CCO,DQM, case managers, dietary, speech, physical, respiratory and wound care staff to facilitate education of Interdisciplinary Teams and processes within IDT meetings as part of theCDI Programs

· Provides education to these service lines and department leadership.

· Provide input on RAC issues relating to clinical documentation.

· Maintains contact with primary care physicians and specialists, and, when indicated, attends doctor visits with mentor and client.
· Administers medications according to established guidelines.
· Provides documentation for all medications given.
· Reviews medication administration records for compliance.
· May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meetings.
· Provides consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment.

· May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation.
· May be required to transport individuals receiving services to planned and/or necessary activities and appointments.
· Promotes health and wellness for individuals and ensures nursing actions and steps are put in place for prevention.
· Performs other duties as assigned.

· Graduation from an accredited nursing school for registered nurses
· Current state RN License
· One year of related experience

Area Director Clinical Documentation Improvement Specialist- (Southern California, CA)

Position Summary:

This is an Area Director level position, which supports multiple long-term acute care hospitals within the program for clinical documentation improvement. This position uses masters level project management, clinical knowledge and knowledge of coded data for documentation requirements to improve overall patient quality, capture severity, acuity and risk of mortality. Expertise required in understanding the clinical documentation required for the completeness of the patient records using a multidisciplinary and interdisciplinary team process. Provides oversight, leads and directs an Area (within a Region) clinical documentation improvement program and staff. Position includes recruiting, hiring and supervising RNs and/or qualified HIM coding professional(s) who are CDI staff. Works with hospital and Regional/District team to reach goals and objectives of the program, which include quality, safety, data integrity and accurate reimbursement. This position functions within the (West) Regional Case Management Department.



Education/License/Certification:

Graduate from an accredited school of nursing (BSN required) and/or medicine and/or experience and Training applicable to clinical and CDI job experience (Physician Assistant, Nurse Practitioner, RHIT) If RN, current licensure to practice as a Registered Nurse in the State of California. Masters Degree preferred. CCDS or CCS optional.


Qualifications:

· Minimum of 6-8 years clinical experience (i.e. inpatient, clinical documentation, and/or case management reviews.

· Prior Clinical Documentation Improvement experience, either as a specialist or as a program manager/leader.

· Expert interpersonal, communication (verbal, non-verbal, and listening skills).

· Understand Adult Learning Theory.

· Understand coding classifications systems such as, but not limited to ICD-9 CM, MS-DRG, APR-DRGs, and HCCs strongly preferred.

· Strong ability to present to multiple physician advisory boards, steering committees and executive staff on the importance of documentation improvement, and effectiveness of CDI and its’ goals and objectives.

· Collect, analyze and interpret data (CDI) for physician endorsement of program initiatives.

· Facilitate understanding, compliance, and completeness in documentation.

· Ability to combine clinical knowledge and business acumen to deliver results with a track record of assessing and capitalizing on opportunities for streamlining operations through broad based experience in project leadership and process reengineering.

· Consistently produce client deliverables faced with multiple project resource constraints.

· Utilize training in Six Sigma methodologies.

· Must exhibit efficiency, collaboration, candor, openness, and results orientation.

· Knowledge of the healthcare revenue cycle.

· Competent computer skills including word processing, spreadsheets, and presentation software.

· Must have strong analytical skills. Demonstrate an understanding of the operations and/or business of Healthcare LTCH, health policy trends, and any applicable regulations related to the responsible practice area.

· Experience with Recovery Audit Contractors (RAC).

Preferred Qualifications:

· Acute care inpatient care experience and extensive knowledge of MS-DRG, APR-DRGs or similar methodology.

· Knowledge of inpatient clinical documentation issues preferred.

· CCDS (certified clinical documentation specialist), CCS (Certified Coding Specialist) optional.


Duties:

· Implement and provide oversight for an Area Clinical Documentation Improvement Program in an organized and standardized manner.

· Recruit, hire and train clinical documentation improvement staff that will function at the District hospitals.

· Mentor and supervise CDI staff.

· Establish effective working relationships with the local and regional staff/teams/leadership.

· Facilitate appropriate clinical documentation to support diagnosis capture.

· Identifies and reviews primary and secondary diagnosis and complications to ensure diagnosis and Identify and review for POA (Present on Admission), Hospital Acquired Conditions (HACs) documentation and initiate a communication clarification process when appropriate with providers.

· Reviews clinical issues with medical coding staff and with physicians to identify those diagnoses that impact severity of illness indicators) for each patient.

· Serves as an expert resource in reviewing all medical records in support of consistent documentation for all payer types (i.e. CMS, Medicare-Advantage, RACs, etc) to ensure complete and accurate diagnosis capture and coding.

· Adheres to the hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.

· Collaborates in the development of programs, initiatives and workflows, which provide alignment with education for internal customers to support clinical documentation guidelines.

· Communicates information timely and effectively with medical center leaders and clinical staff.

· Works withCase Management Corporate and Regional Senior Directors to develop, implement and monitor departmental policies and procedures that support organizational goals, business objectives, regulatory needs and requirements.

· Conducts quality assurance reviews on the CDI processes and functions, resulting in the reporting of any corrective action.

· Collaborates with the interdisciplinary contacts of the medical centers; CCO,DQM, case managers, dietary, speech, physical, respiratory and wound care staff to facilitate education of Interdisciplinary Teams and processes within IDT meetings as part of theCDI Programs

· Provides education to these service lines and department leadership.

· Conducts data and root cause analysis; provides feedback and shares findings on the analysis to leaders, local regional management and medical team.

· Leads the “Query process” to medical staff for accurate clear documentation in the patient’s medical records. Monitor and track verbal and written queries and produce reports as required.

· Provide input on RAC issues relating to clinical documentation.

· Serves on committees and work groups as needed and as appropriate including but not limited to the Regional CDI Executive Steering Committee.

Position Summary:

This is an Area Director level position, which supports multiple long-term acute care hospitals within the program for clinical documentation improvement. This position uses masters level project management, clinical knowledge and knowledge of coded data for documentation requirements to improve overall patient quality, capture severity, acuity and risk of mortality. Expertise required in understanding the clinical documentation required for the completeness of the patient records using a multidisciplinary and interdisciplinary team process. Provides oversight, leads and directs an Area (within a Region) clinical documentation improvement program and staff. Position includes recruiting, hiring and supervising RNs and/or qualified HIM coding professional(s) who are CDI staff. Works with hospital and Regional/District team to reach goals and objectives of the program, which include quality, safety, data integrity and accurate reimbursement. This position functions within the (West) Regional Case Management Department.



Education/License/Certification:

Graduate from an accredited school of nursing (BSN required) and/or medicine and/or experience and Training applicable to clinical and CDI job experience (Physician Assistant, Nurse Practitioner, RHIT) If RN, current licensure to practice as a Registered Nurse in the State of California. Masters Degree preferred. CCDS or CCS optional.


Qualifications:

· Minimum of 6-8 years clinical experience (i.e. inpatient, clinical documentation, and/or case management reviews.

· Prior Clinical Documentation Improvement experience, either as a specialist or as a program manager/leader.

· Expert interpersonal, communication (verbal, non-verbal, and listening skills).

· Understand Adult Learning Theory.

· Understand coding classifications systems such as, but not limited to ICD-9 CM, MS-DRG, APR-DRGs, and HCCs strongly preferred.

· Strong ability to present to multiple physician advisory boards, steering committees and executive staff on the importance of documentation improvement, and effectiveness of CDI and its’ goals and objectives.

· Collect, analyze and interpret data (CDI) for physician endorsement of program initiatives.

· Facilitate understanding, compliance, and completeness in documentation.

· Ability to combine clinical knowledge and business acumen to deliver results with a track record of assessing and capitalizing on opportunities for streamlining operations through broad based experience in project leadership and process reengineering.

· Consistently produce client deliverables faced with multiple project resource constraints.

· Utilize training in Six Sigma methodologies.

· Must exhibit efficiency, collaboration, candor, openness, and results orientation.

· Knowledge of the healthcare revenue cycle.

· Competent computer skills including word processing, spreadsheets, and presentation software.

Preferred Qualifications:

Duties:

· Implement and provide oversight for an Area Clinical Documentation Improvement Program in an organized and standardized manner.

· Recruit, hire and train clinical documentation improvement staff that will function at the District hospitals.

· Mentor and supervise CDI staff.

· Establish effective working relationships with the local and regional staff/teams/leadership.

· Facilitate appropriate clinical documentation to support diagnosis capture.

· Identifies and reviews primary and secondary diagnosis and complications to ensure diagnosis and Identify and review for POA (Present on Admission), Hospital Acquired Conditions (HACs) documentation and initiate a communication clarification process when appropriate with providers.

· Reviews clinical issues with medical coding staff and with physicians to identify those diagnoses that impact severity of illness indicators) for each patient.

· Serves as an expert resource in reviewing all medical records in support of consistent documentation for all payer types (i.e. CMS, Medicare-Advantage, RACs, etc) to ensure complete and accurate diagnosis capture and coding.

· Adheres to the hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.

· Collaborates in the development of programs, initiatives and workflows, which provide alignment with education for internal customers to support clinical documentation guidelines.

· Communicates information timely and effectively with medical center leaders and clinical staff.

· Works withCase Management Corporate and Regional Senior Directors to develop, implement and monitor departmental policies and procedures that support organizational goals, business objectives, regulatory needs and requirements.

· Provides education to these service lines and department leadership.

· Conducts data and root cause analysis; provides feedback and shares findings on the analysis to leaders, local regional management and medical team.

· Leads the “Query process” to medical staff for accurate clear documentation in the patient’s medical records. Monitor and track verbal and written queries and produce reports as required.

· Provide input on RAC issues relating to clinical documentation.

· Serves on committees and work groups as needed and as appropriate including but not limited to the Regional CDI Executive Steering Committee.

8. Assist in employee training as necessary.

Area DirectorClinical Documentation Improvement Specialist (Southern California, CA)

Position Summary:

This is an Area Director level position, which supports multiple long-term acute care hospitals within the program for clinical documentation improvement. This position uses masters level project management, clinical knowledge and knowledge of coded data for documentation requirements to improve overall patient quality, capture severity, acuity and risk of mortality. Expertise required in understanding the clinical documentation required for the completeness of the patient records using a multidisciplinary and interdisciplinary team process. Provides oversight, leads and directs an Area (within a Region) clinical documentation improvement program and staff. Position includes recruiting, hiring and supervising RNs and/or qualified HIM coding professional(s) who are CDI staff. Works with hospital and Regional/District team to reach goals and objectives of the program, which include quality, safety, data integrity and accurate reimbursement. This position functions within the (West) Regional Case Management Department.



Education/License/Certification:

Graduate from an accredited school of nursing (BSN required) and/or medicine and/or experience and Training applicable to clinical and CDI job experience (Physician Assistant, Nurse Practitioner, RHIT) If RN, current licensure to practice as a Registered Nurse in the State of California. Masters Degree preferred. CCDS or CCS optional.


Qualifications:

· Minimum of 6-8 years clinical experience (i.e. inpatient, clinical documentation, and/or case management reviews.

· Prior Clinical Documentation Improvement experience, either as a specialist or as a program manager/leader.

· Expert interpersonal, communication (verbal, non-verbal, and listening skills).

· Understand Adult Learning Theory.

· Understand coding classifications systems such as, but not limited to ICD-9 CM, MS-DRG, APR-DRGs, and HCCs strongly preferred.

· Strong ability to present to multiple physician advisory boards, steering committees and executive staff on the importance of documentation improvement, and effectiveness of CDI and its’ goals and objectives.

· Collect, analyze and interpret data (CDI) for physician endorsement of program initiatives.

· Facilitate understanding, compliance, and completeness in documentation.

· Ability to combine clinical knowledge and business acumen to deliver results with a track record of assessing and capitalizing on opportunities for streamlining operations through broad based experience in project leadership and process reengineering.

· Consistently produce client deliverables faced with multiple project resource constraints.

· Utilize training in Six Sigma methodologies.

· Must exhibit efficiency, collaboration, candor, openness, and results orientation.

· Knowledge of the healthcare revenue cycle.

· Competent computer skills including word processing, spreadsheets, and presentation software.

· Must have strong analytical skills. Demonstrate an understanding of the operations and/or business of Healthcare LTCH, health policy trends, and any applicable regulations related to the responsible practice area.

· Experience with Recovery Audit Contractors (RAC).

Preferred Qualifications:

· Acute care inpatient care experience and extensive knowledge of MS-DRG, APR-DRGs or similar methodology.

· Knowledge of inpatient clinical documentation issues preferred.

· CCDS (certified clinical documentation specialist), CCS (Certified Coding Specialist) optional.


Duties:

· Implement and provide oversight for an Area Clinical Documentation Improvement Program in an organized and standardized manner.

· Recruit, hire and train clinical documentation improvement staff that will function at the District hospitals.

· Mentor and supervise CDI staff.

· Establish effective working relationships with the local and regional staff/teams/leadership.

· Facilitate appropriate clinical documentation to support diagnosis capture.

· Identifies and reviews primary and secondary diagnosis and complications to ensure diagnosis and Identify and review for POA (Present on Admission), Hospital Acquired Conditions (HACs) documentation and initiate a communication clarification process when appropriate with providers.

· Reviews clinical issues with medical coding staff and with physicians to identify those diagnoses that impact severity of illness indicators) for each patient.

· Serves as an expert resource in reviewing all medical records in support of consistent documentation for all payer types (i.e. CMS, Medicare-Advantage, RACs, etc) to ensure complete and accurate diagnosis capture and coding.

· Adheres to the hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.

· Collaborates in the development of programs, initiatives and workflows, which provide alignment with education for internal customers to support clinical documentation guidelines.

· Communicates information timely and effectively with medical center leaders and clinical staff.

· Works withCase Management Corporate and Regional Senior Directors to develop, implement and monitor departmental policies and procedures that support organizational goals, business objectives, regulatory needs and requirements.

· Conducts quality assurance reviews on the CDI processes and functions, resulting in the reporting of any corrective action.

· Collaborates with the interdisciplinary contacts of the medical centers; CCO,DQM, case managers, dietary, speech, physical, respiratory and wound care staff to facilitate education of Interdisciplinary Teams and processes within IDT meetings as part of theCDI Programs

· Provides education to these service lines and department leadership.

· Conducts data and root cause analysis; provides feedback and shares findings on the analysis to leaders, local regional management and medical team.

· Leads the “Query process” to medical staff for accurate clear documentation in the patient’s medical records. Monitor and track verbal and written queries and produce reports as required.

· Provide input on RAC issues relating to clinical documentation.

· Serves on committees and work groups as needed and as appropriate including but not limited to the Regional CDI Executive Steering Committee.
