This management position is responsible for responding to service requests and following up with customers and their families . You will be responsible for the initial home or facility visit, needs assessment and care plan development. You will conduct follow-up visits throughout duration of services to ensure that the highest level of care is maintained. A key component of this position requires a comfort level in sales and marketing ElderSafe services both to individuals and health care partners. Travel in the North Simcoe Muskoka area will be required.

JOB SUMMARY:
Leisureworld Caregiving Centre Creedan Valley is home to 95 residents and currently has an opening for a permanent casual Registered Nurse. The initial assignment is to fill a temporary full time position for night shifts.
RESPONSIBILITIES (But not limited to):
Responsible for assessing, planning, implementing and evaluating physical, psychological, emotional, spiritual, cultural and social needs of the resident
Validates the plan of care, goals and objectives with the resident/family
Provides effective guidance and supervision for the Registered Practical Nurse (RPN) and the Personal Support Worker (PSW)
Ensures that documentation standards are reflected in the residents chart
Liaises with Ministry of Health Compliance Advisors and Consultants, social workers, physicians, psychiatrists, the Coroner and appropriate health care facilities
QUALIFICATIONS:
Current certificate of registration from the College of Nurses of Ontario
Proven organizational, observational, interpersonal and communication skills
Long-term care experience is preferred
EDUCATION:
Bachelor of Science Nursing or equivalent
Certificate in gerontology nursing preferred
Current BCLS or Heart Saver Certificate an asset

Residential Hospice provides people facing a life threatening illness with medical and non-medical support in a home like environment.
Responsibilities:
In collaboration with residents, families and members of the formal and informal care team, perform assessments and plan, implement and evaluate care
Provide direct care to Hospice residents and families
Assist staff in enhancing knowledge and skills to provide quality palliative care
Participate in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees
Act as a clinical support to nurses, managers and interdisciplinary staff on care issues and problem solving to ensure the promotion of client focused care
Perform other relevant duties as required
Qualifications:
Current registration with the College of Nurses of Ontario is required
Hospice Palliative Care certificate or course is preferred
Excellent verbal and written communication skills are required
Ability to deal with others effectively in an interdisciplinary team
Demonstrated leadership skills, problem solving and decision making skills
Customer focused availability to meet client needs is required
Availability to work all shifts including days, evenings, nights and week-ends is required
