Residential Hospice provides people facing a life threatening illness with medical and non-medical support in a home like environment.
Responsibilities:
• In collaboration with residents, families and members of the formal and informal care team, perform assessments and plan, implement and evaluate care
• Provide direct care to Hospice residents and families
• Assist staff in enhancing knowledge and skills to provide quality palliative care
• Participate in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees
• Act as a clinical support to nurses, managers and interdisciplinary staff on care issues and problem solving to ensure the promotion of client focused care
• Perform other relevant duties as required
Qualifications:
• Current registration with the College of Nurses of Ontario is required
• Hospice Palliative Care certificate or course is preferred
• Excellent verbal and written communication skills are required
• Ability to deal with others effectively in an interdisciplinary team
• Demonstrated leadership skills, problem solving and decision making skills
• Customer focused availability to meet client needs is required
Availability to work all shifts including days, evenings, nights and week-ends is required
